Physical Rehabilitation Outpatient
Who can be helped by Physical Rehabilitation Services?
Physical rehabilitation is provided for patients who have suffered:
- Stroke (CVA)
- Brain Injury
- Spinal Cord Injury or Dysfunctions
- Neurological Disorders, including Parkinson’s, Multiple Sclerosis, and Polyneuropathy
- Femur Fractures
- Orthopedic Conditions, such as Knee and Hip Replacements, and Fractures
- Amputation
- Major Multiple Trauma
- Cardiac Conditions
- Pulmonary Conditions
- Pain Syndromes
- Medically Complex Conditions
- Arthritic Impairments, including Rheumatoid Arthritis, and Osteoarthritis
- Other Debilitating Conditions
Our success rate is higher for patients who:
- Have the potential to improve functional skills
- Are able to tolerate a comprehensive rehab program, 5 days per week of Physical, Occupational, and/or Speech Therapy
- Are motivated to participate in the program
- Require 24-hour rehabilitation nursing care
Physicians will be interested to know that the excellent patient/staff ratios ensure truly individualized care for their patients. In addition, it is our rehab team’s priority to keep referring physicians informed on the progress of their patients. Upon discharge the attending physician’s discharge summary is sent to the referring physician.
Making a Referral
Referrals may be made 24 hours a day, 7 days a week. Pre-admission assessments to determine a patient’s eligibility for admission are provided at no charge. Medicare and most insurance plans are accepted.
